Diagnosis codes (ICD-10-CM)

Review the ICD-10-CM codes Quillentra suggests from your note, each with the text that supports it, then accept, replace, remove or add codes.

On this page· 3 sections
  1. What each suggestion shows
  2. Review the codes
  3. Copy, export and signing

After a clinical note is written, Quillentra reads the note (not the transcript) and suggests the ICD-10-CM diagnosis codes it supports. They appear under the note in Diagnosis codes. On wider screens, select Codes in the document toolbar to jump there. Letters, patient documents and forms don’t get codes.

What each suggestion shows

  • The code and its official description.
  • The text in your note that supports it, and the section it’s in. If you later edit that text out, the code is flagged so you can check it still applies.
  • Unspecified when the code is an “unspecified” one, with more specific codes and what the documentation would need to state for each.

Every suggestion is checked against the official CMS ICD-10-CM code set for the current fiscal year. Codes that don’t exist, and category codes that can’t be billed, are never shown. If a visit’s date of service falls outside that fiscal year, you’ll see a reminder to check the codes against the code set in effect on that date.

Review the codes

  • Accept a code you agree with.
  • Use one of the more specific codes listed, or select Replace to search for any billable code.
  • Remove a code that doesn’t apply. Removed codes are listed under Removed, where you can restore them.
  • Add a code searches the code set by code (for example F41) or by description (for example panic disorder).

If you edit the note after codes were suggested, select Re-check. Your accepted, removed and added codes are kept, and a removed code isn’t suggested again.

Copy, export and signing

  • Only accepted codes leave Quillentra: Copy accepted, Copy all and PDF and Word exports include them at the end of the note.
  • Signing locks the codes with the note. To change them, amend the note.
  • Every suggestion run and every decision is recorded in your practice’s audit log, without the codes themselves.

Suggestions support your coding; they don’t replace it. You’re responsible for the codes you submit. Quillentra doesn’t suggest CPT or E/M codes here; you can ask the AI assistant with /codes for suggestions to verify.
